Billing Information

Payments can be processed as follows:


In the Event of an Employee Injury

Get your injured employee medical attention immediately by:

  • Calling 911 (if needed).
  • Providing first aid (if trained staff is available).
  • Taking or sending the injured employee to a State Fund Medical Provider Network (MPN) facility if he or she has not predesignated a physician.
  • Employees who have predesignated can be sent to their personal physician.

Claim Forms

Workers Compensation Claim Form – to be completed out by the employee and employer

Employer’s Report of Occupational Injury or Illness– to be completed by the employer/policyholder


Claim Reporting

Report your workers compensation claim directly to State Fund by calling (888) 782-8338, option 1 (for English) or 2 (for Spanish) then 3 (for claims) and then 3 again (for a new claim)

State Fund’s experienced claims representatives are available Monday through Friday, except holidays, from 7:30 a.m. to 5:30 p.m. Pacific Standard Time (PST) to:

  • Complete the Employer’s Report of Occupational Injury or Illness form with you over the phone.
  • Eliminate any paperwork on your part.
  • Immediately assign a claim number to your case.
  • Mail written confirmation of your report to you the next working day or fax a copy to you the same day.
  • Immediately forward your claim to the appropriate State Fund Regional Office.
  • Refer you to a physician who has experience with occupational injuries.
  • Find a physician located closest to the site of the industrial accident or the injured worker’s home.
  • Provide assistance in English and Spanish.

For injuries occurring before or after normal business hours, please log on to State Fund Online to submit a claim electronically or fax the completed claims documents to our Claims Reporting Center at

(800) 371-5905.

Additionally State Fund has Medical Access Assistants available Monday through Saturday 7 a.m. to 8 p.m., by phone at (855) 521-7082 or by fax at (571) 446-2070 should you need assistance locating an available MPN physician or with scheduling a medical appointment. You may also email your appointment request to statefundMPNMAA@harborsys.com.
